I made 5-6 attempts at getting a 1 x 2. I eventually opted to go another route. I was able to get a refund for my unsuccessful attempts.
It started with calling the FCC help desk and explaining what I wanted. They sent me an email with the procedures which included scanning and providing to them the dismissal letters, along with a request that mentioned the file numbers associated with the applications.
I did that and they responded by sending me an ACH form, I think it was a SF-3881. I filled it out and provided my banking information Routing code and account number and emailed it back to them at the email address provided on the form.
My refunds came about 1 week later (approximately)... I hope this helps someone else in the same situation.
